Supportive Services / Case Management

Cornerstone partners with local churches and organizations to help meet the physical, emotional, and spiritual needs of those we are assisting. Our intake process includes providing a strength-based assessment of those seeking help, as well as referrals to Cornerstone programs and other community services. We work to individualize a plan with each client based on their personal assets and goals. Because we value every person, our goal is to provide every participant the best supportive services possible in order that they might become all that God intends.

Clients are served by appointment at Cornerstone after they receive a referral from a Cornerstone Network Partner church or organization. Along with ongoing support and coaching with a case manager, Cornerstone may provide emergency clothing and food to clients in need. These services are available by appointment during the hours of 9am-12pm and 1-3 pm, Monday, Wednesday, and Friday.

Clients must bring:

  • Referral letter from a Cornerstone Network Partner

  • Photo ID
